2021/22 Parking Reimbursement for Downtown, Chinatown, Mission, and Ft. Mason
Faculty eligible for parking reimbursements must use the forms below and submit the following by May 31, 2022:
- the signature page
- the completed spreadsheet
- all original receipts, or scan receipts and retain originals
Faculty should apply for parking reimbursement either by emailing or by paper mailing:
- emailing your forms and receipts to aft@aft2121.org,
- sending hard copies of forms and the original receipts to: AFT Local 2121, 311 Miramar Ave, San Francisco, CA 94112
We can’t guarantee that late parking forms will be processed. Be sure to download and open the spreadsheet on a computer. Type each receipt’s dollar amount into the correct month/day. The spreadsheet will automatically calculate totals, saving you time and lightening the burden on our volunteer parking receipt verification team.

In order to be reimbursed, you must fill out this excel sheet to automatically calculate your totals. Print and submit to your campus rep with receipts collated in order by date and time, and the signature form (below) indicating your work schedule.
